Immigration exams must be performed by a Civil Surgeon for form I-693. Trinity’s Civil Surgeon is Dr. Howard Reeve.

What to bring:

• Translator: Fluent in English and your native language (must be over 18)
• Federally issues ID such as Passport, Work Visa, or Driver’s License with a star
• Vaccination Records
  -Must be current and in English- If not in English must be professionally translated.

  – Exams are cash or credit card only. Payment plans and insurance are not accepted for this service. Vaccination payments will be separate from exam and lab costs.
