Why Credential?
Trinity Health is legally responsible for verifying that individuals providing care at its facilities are appropriately qualified and competent to provide patient care. The Joint Commission, The Centers for Medicare and Medicaid Services (CMS), National Committee on Quality Assurance (NCQA), the N.D. Department of Health and other oversight organizations require that members of the medical and allied health staff be credentialed and privileged before working in the facility.
What is Credentialing?
Credentialing is a term that usually encompasses two separate processes: credentialing and privileging. Credentialing is the primary source verification of a healthcare practitioner’s education, training, work experience, license, etc. Privileging is the vetting of an individual wanting to perform a specific procedure or specific set of privileges based on documented competence in the specialty in which privileges are requested.
Credentialing Process
Thank you for your interest in gaining medical staff privileges to practice at Trinity Health. For your convenience, the application process is done through an online application. The following are the steps required for the application process:
Step 1: The provider completes the Medical Staff Eligibility Form.
Step 2: Once the Eligibility Form is reviewed to ensure the minimum criteria to apply for membership is met, the Medical Staff Request for Appointment will be emailed to the provider.
Step 3: The Medical Staff Office will complete Primary Source Verification.
Step 4: Approval and Enrollment
To receive a link to the Medical Staff Eligibility Form, please contact the Medical Staff Services Office. Completion of the Medical Staff Eligibility Form in no way obligates the hospital and/or medical staff to grant medical staff membership or privileges.
When starting the credentialing process, please ensure you have applied for:
- North Dakota license
- Federal DEA registration with a North Dakota address
These need to be pending at the time of your application for medical staff privileges and approved before privileges can be granted.
Please take time to thoroughly review the electronic application and supporting documentation that you will receive with it. With the application link, you will receive the Medical Staff Bylaws, Rules and Regulations, and other related documents.
You can help ensure timely processing of your application by confirming it is completed fully and assist the Medical Staff Services Office as requested in obtaining primary source verification regarding your education, previous training/employment, insurance coverage, peer reviews, etc. Your help will expedite the process.
