The Trinity Health Center for Diabetes Education will host a pair of informational sessions for people interested in joining its diabetes prevention program, Weigh 2 Change.
Weigh 2 Change is a yearlong, CDC-approved program that helps people with prediabetes take control of their health through nutrition, exercise, group support, and the use of trained lifestyle coaches. Participants must be 18 years or older, be at risk for – or have – prediabetes, and be overweight. They can’t be pregnant or have diabetes.
Two “Session Zero” meetings have been scheduled where people can learn about the program, complete a Readiness-to-Change questionnaire, and decide whether the program is right for them. The free sessions will be held January 8 and 22 from 4 to 5 p.m. in the Trinity Health Community Conference Room at Town & Country Center.
People who decide to commit to the program and are not covered by Medicare will be charged a fee of $220 to cover educational sessions and materials for the entire year. For more information, call the Trinity Health Center for Diabetes Education at 857-5268.
