
Transforming Gratitude into Action with the Grateful Patient Program
Trinity Health Foundation is excited to introduce the Grateful Patient Program where patients and families are invited to celebrate a great experience at Trinity Health through sharing stories or making a financial contribution.
How the Grateful Patient Program Works:
- Grateful Patient Program brochures will be available to patients/families at nursing stations throughout the hospital.
- Brochures will provide patients and families the ability to transcribe their personal story connected to the care they received at Trinity Health.
- Once completed, patients may return the brochure to the Foundation office or email their story to [email protected].
- In addition to sharing stories, patients/families wishing to express their gratitude through a financial contribution may use the brochure to complete that donation.
- Donations made to Trinity Health Foundation through the Grateful Patient Program directly support our ongoing initiatives to enhance patient care, fund staff training and provide essential resources.
- On the back of the brochure, patients will find a section to provide their information for making a financial gift. Completed forms can be returned to Trinity Health Foundation, PO Box 5020, Minot, ND 58702.
The Grateful Patient Program not only empowers patients to express their appreciation for the care they received but also provides an opportunity for them to give back in meaningful ways.
