This program provides a safe and effective bridge from the structured rehabilitative environment to the activities of daily living. Physical reconditioning is provided as a continuum of healthcare following physical therapy, occupational therapy or cardiac rehabilitation. For example, IEPs are available for individuals with specific health needs in the areas of orthopedic and musculoskeletal injuries, i.e. lower back pain, osteoporosis or joint disorder.

For individuals whose healthcare professional has determined than an exercise program would be beneficial in treating their disease or disability. IEPs are designed within the scope of the physician's recommendations and restrictions, as well as the patient's risk factors, individual goals and interests. For example, IEPs are available with specific health needs in the areas of:

• weight management
