Trinity's RehabCare Center is an inpatient rehabilitation program dedicated to helping people restore their independence and return to a more productive lifestyle following an illness, injury or surgery. Our team of specialized professionals provides a comprehensive range of services to assist each individual in obtaining the highest level of functioning possible.
Rehabilitation Medicine is a medical specialty that focuses on the evaluation and functional restoration of patients with disabilities or limitations in functional abilities. Trinity's RehabCare program is built on a team approach that helps patients set realistic goals and earn a sense of confidence in their ability to manage their lives once they have left the RehabCare setting. Therapeutic plans are developed by all of the rehabilitation disciplines to meet the functional, vocational and psychological goals of each patient. These disciplines include:

The Rehabilitation Environment
The RehabCare Center provides a cheerful and functional atmosphere where each patient is encouraged to strive for his/her highest level of achievable independence. Patients move throughout the unit as independently as possible. A day room serves as the patient's community dining room as well as a setting for recreational activities and family meetings. Patient rooms are furnished with phones and television sets.
Family & Patient Education
The Rehab team will inform the family or caregiver of appropriate opportunities to participate in the patient's therapy sessions. Depending on the patient's needs, education may also include activities such as community re-entry outings and passes as well as the general education provided on an on-going basis by all staff members.
The RehabCare staff recognizes the importance of patient outings, not only as a recreational activity but also as a strategy for learning compensation for lost skills. Therapeutic outings are coordinated by the staff and incorporate physical therapy, occupational therapy and communication goals designed to maximize the patient's level of independence.
Therapeutic Timed Absences (TTA)
With family, we provide needed recreation for the patient and are useful in consolidation of new skills. When a therapist does not accompany the patient, written goals for the TTA will be given and reviewed with the family member who is escorting the patient. The family member will be asked to complete an observation report when the patient returns to the hospital.
The RehabCare team meets weekly to review the patient's progress and to make appropriate adjustments in short- and long-term goals. A discharge date is set early in the patient's program as required by Medicare and third party payers. However, this date is tentative and subject to change based on the patient's progress through the program. Family conferences are scheduled, preferably within the first week of the patient's admission. The goal is to encourage open communication between patient, family and staff to help set realistic goals and to plan for the patient's discharge from the hospital.
As part of our commitment to the recovery and adjustment of the patient and family, follow-up assistance is provided after discharge. Patients/families are contacted by the center's medical social worker within two weeks and three months after discharge. These calls offer an opportunity to discuss progress and whatever concerns may have arisen. Individual sessions may be arranged with the social worker to resolve adjustment issues. Of course, patients and families are free to contact the Center at any time.
Patients are required to meet certain criteria before they may be admitted to the RehabCare Center. We provide services for patients 18 and older.
Patient requires more than one form of therapy
Patient is medically stable and demonstrates sufficient endurance and ability to participate in a comprehensive rehabilitation program.
Patient and his/her family demonstrates a willingness to be involved in a rehabilitative program.
Patient has the potential for making significant gains in functional capability.
