Paid Time Off - Giving You Time:
Personal time off with pay is called Paid Time Off (PTO). This time may be used at the employee's discretion, for personal reasons, according to policy. Accruals begin with employment and are available for use the first day of the pay period following 90 days of employment. PTO accrual rates increase with longevity. Use of PTO is encouraged, however, carry-over is permitted. PTO is a wonderful benefit that FT and PT employees earn, based on hours worked. LPT employees will earn PTO on hours worked in excess of regularly scheduled hour of work. Accrual rates vary based on employee status.
An example of how PTO works is if you are a full-time employee for 1 year (working 40 hours per week for 52 weeks) you would accrue .0577 hours of PTO per hour worked. After 2,080 hours this equates to 3 weeks or 120 hours of PTO. The rate increases at 2081, 10401 and 31201 hours.
Extended sick time accrues from the date of hire, separate from the PTO accruals and is available for use the first day of the pay period following 90 days of employment. Sick leave is designed to assist the employee during times of illness. 8 hours of Extended Leave are accrued for each 260 hours worked.
Health Insurance - We've Got You Covered:
Trinity Health offers employees health insurance through Blue Cross-Blue Shield of North Dakota. Employees working at least 20 hours per week are eligible for health insurance coverage through Trinity Health.
Trinity offers a single, single plus dependant, and family health care plan with Trinity sharing in the cost. Eligibility is the 1st or 16th of the month following date of hire, whichever is closest.Premiums (per pay period):

Copay: $25 office visit
In-Network Deductible: $500/$750/$1,000
In-Network Co-Insurance: $1,500/$2250/$3,000
In-Network Out of Pocket Maximum: $2,000/$3,000/$4,000
$200 Annual Benefit for Preventive Services - deductible/coinsurance waived (routine physical exam, cholesterol screening, urinalysis testing, blood sugar screening, hemoglobin testing)
Preventive services covered in addition to your $200 preventive maximum - deductible/coinsurance waived (mammogram, pap smear, fecal occult blood testing, prostate cancer screening services - frequency of services may vary by age group)
Immunizations covered at 100%
Eligible Dependent Children under age 26 who are not eligible to enroll in an employer sponsored health plan other than a group plan of the parent may continue coverage until the end of the month in which the children become age 26. The member must notify employer if the dependent children become eligible to enroll in another employer sponsored plan. For additional information, please contact Human Resources.
Dental Insurance - Something to Smile About:
In addition to its health benefits package, Trinity Health offers employees dental coverage under their group health contract. Employees may also obtain family dental coverage for an additional charge.
Dental coverage begins the same time your health benefits begin. Dental coverage through Trinity Health may also be obtained as a stand-alone benefit, even if another insurance carrier is providing your health insurance. Eligibility is the 1st of 16th of the month following date of hire, whichever is closest.
Premiums (per pay period):
Single = $5.20 per pay period
Family = $23.83 per pay period
Cleaning Co-pay: $10/cleaning (4 per year)
Diagnostic and Preventative: 2 cleanings annually, deductible waived
Restorative: Covers 80% on fillings, 50% on crowns
Deductible: $50 per person, $100 family
Benefit Max per year: $1,000 per member
After one year of employment, Trinity Health employees, working at least 20 hours per week will receive life insurance benefits equal to 1 ½ times their base pay at no cost to them. Optional life coverage as well as dependent coverage, at the employees expense, is available if the employee is interested.
Long Term Disability Insurance:
Following one year of continuous employment, Trinity Health employees working at least 20 hours per week are covered by long-term disability insurance at no cost to the employee. A benefit of 60% of monthly income, not to exceed $3,000, continues until the age of 65 or until the employee returns to work or the disability ceases to qualify the person for coverage.
Holidays - Celebrate the Holidays!:
At Trinity Health we observe the following six legal holidays. New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. Eligibility is after 90 days. Holiday pay is built in to PTO accrual.
Tuition Assistance - Education is Important:
Trinity Health provides tuition assistance up to $500.00 per school year to all employees working at least 20 hours per week. Courses must be job-related and approval is required before class registration. For details, contact HR.
Trinity Health Foundation offers many nursing scholarships each year for both RN's and LPN's. Any student who is accepted into a nursing program is eligible to apply for the scholarship/ loan program. The program may allow for up to $2,000 per semester per student PLUS a $250 stipend each month is employed while attending school. For detailed information on this program, contact the Human Resources Department.
Direct Deposit - Convenience For You:
Trinity Health will automatically deposit your paycheck every two (2) weeks into either a checking or savings account of your choice. Employees can view or print their pay voucher each payday reflecting their earnings for the current pay period and year to date.
TriniPlus 401(K) - Investing With You:
Employees of Trinity Health, age 21 or older, are eligible to participate in Trinity's 401(k) retirement plan. Our TriniPlus 401(k) plan allows employees the chance to invest on a tax-deferred basis for long-term goals, including retirement. Even better, Trinity will immediately begin to contribute funds to this program for those employees participating. Trinity matches 25% of the amount of your voluntary pre-tax contributions, up to a maximum of 6% of your pay. The 401k employer matching contributions are fully vested after 1 year. Employees can generally withdraw funds when they leave Trinity Health or reach age 59 ½.
Pension Plan - Saving For You:
Trinity Health may elect each year to make a discretionary contribution to the TriniPlus Profit Sharing plan. This plan is open to all employees after one year of employment, provided they are 21 years of age or older and work a minimum of 1,000 hours per year. Employees are fully vested after 6 years of vesting service (1000 hours per year) in any contributions made to this plan once they are eligible.
Voluntary Benefits1. Short Term Disability / Voluntary Income Protection:
Trinity Health employees who work a minimum of 20 hours per week may purchase short-term disability at a group rate. Benefits are paid for a disability resulting from an accident or illness, work related or not. Short-term disability insurance replaces up to 60% of gross income and payment is based upon the schedule you choose (3 options). Rates are determined by your benefits selected and salary. There are no health questions asked if you apply when you're eligible. The benefit is post-tax and only available during employment.2. CancerCare Elite Insurance:
Trinity Health employees who work a minimum of 20 hours per week, are under the age of 65, and qualify, may purchase this voluntary, pre-tax insurance at a group rate. This insurance provides various benefits to a cancer patient including family lodging, airfare, experimental treatment, wig and hairpiece, at home recovery benefit and many more benefits that health insurance does not cover. It also covers a $75 per person/per year Wellness Benefit. This insurance is portable.3. Hospital Indemnity Plan:
Trinity Health employees who work a minimum of 20 hours per week are eligible to enroll in this voluntary post-tax benefit that pays members up to $1000 when they are hospitalized for 24 hours. It also covers a $75 per person/per year Wellness Benefit. This insurance is portable.4. Long-Term Care Insurance:
All Trinity Health employees and family members are eligible to apply for this voluntary benefit which may help if you become disabled due to an injury, a chronic medical condition, the effects of aging or a cognitive impairment. This insurance is portable.5. Long-Term Disability Buy-Up:
The core Long-Term Disability benefit is provided by Trinity Health according to the information listed above. The benefit amounts to 60% of monthly income up to $3,000/month. Starting in 2013, we have introduced a buy up feature for those qualified and wish to take advantage of the increased benefit. Trinity Health will continue to provide the core benefit as we have in the past however the qualified participant will have the option to buy-up the benefit to 60% of monthly income up to $6,000 per month.6. Purchasing Power:
To qualify for this voluntary program, you must be employed 9 months, earn a minimum annual income of $16,000, and work at least 20 hours per week. Purchasing Power is an employee purchase program that gives budget-conscious individuals access to buy brand-name computers, electronics, home appliances and furniture via convenient payroll deduction. Purchasing Power is an exclusive voluntary benefit offered by Trinity Health that is not available to the public. The program offers an upfront price and there are no credit checks or upfront cash required. Items purchased are delivered right to your front door.
Flexible Spending - Flexibility For You:
Flexible Spending is available to staff who work a minimum of 1,040 hours per year. This benefit puts additional dollars in employees' pockets by letting them set aside dollars on a pre-tax basis to pay for out-of-pocket medical expenses or dependent adult or child care expenses. Out-of-pocket medical expenses include co-payments, deductibles, prescription drugs, chiropractic care, eyeglasses and out-of pocket dental expenses. In 2013, you are able to flex $2,500 for medical spending (e.g., co-pays, deductibles, co-insurance, dental, vision, prescription drugs, hospital services, lasik eye surgery) and $5,000 for dependent care/daycare expenses for children up to age 13. This is administered through Discovery Benefits.
Trinity Discounts - Enjoy!
Employees, along with their spouse and dependents are provided certain medical service discounts by Trinity Health. Employees and volunteers are offered a variety of community discounts. Discounts available can be found in Trinity's Handbook under Other Benefits and Services and Discounts.
Trinity employees are rewarded for working PM, nights and weekend shifts by receiving an added pay differential to their hourly pay rate. Those differentials are currently as follows:
Weekday differentials include: $.75 PM shift, $1.00 night shift
Weekend differentials include: $.75 days, $1.50 PM, and $1.75 nights
Longevity Incentive Program
TH may give you credit for previous service at a hospital affiliated with the Voluntary Hospital Association (VHA). If you've worked for a VHA (e.g., ND hospitals include: Altru, Jamestown Hospital, Essential Health, MedCenter One, Sanford Health), Trinity may give you credit for PTO time and up to 12 days of sick time if you have had less than 3 months break in service from that facility prior to beginning your employment at TH. HR must be notified of this upon hire and you will required to provide proof with a letter from the VHA employer to include dates of employment, your employment status during your employment and what your sick balance was at termination. You will still follow the normal waiting periods to begin using those benefits.
Employee Assistance Program (EAP)
An employee's work performance may be affected by various problems including physical, mental or emotional illness, finances, marital or family distress, alcoholism, drug abuse, legal problems or other concerns. Trinity offers the employee and their family members the opportunity to speak confidentially with a member of Pastoral Care. Referrals may be made to community agencies for long-term care or specialized assistance.
