  • Paid Time Off - Giving You Time:

    Personal time off with pay is called Paid Time Off (PTO). This time may be used at the employee's discretion, for personal reasons, according to policy. Accruals begin with employment and are available for use after 90 days of employment. PTO accrual rates increase with longevity. Use of PTO is encouraged, however, carry-over is permitted. PTO is a wonderful benefit that FT and PT employees earn, based on hours worked. Accrual rates vary based on employee status.

    An example of how PTO works is if you are a full-time employee for 1 year (working 40 hours per week for 52 weeks) you would accrue .0577 hours of PTO per hour worked. After 2,080 hours this equates to 3 weeks or 120 hours of PTO. The rate increases at 2081, 10401 and 31201 hours.

  • Extended Leave:

    Extended sick time accrues from the date of hire, separate from the PTO accruals and is available for use after 90 days of employment. Sick leave is designed to assist the employee during times of illness. 8 hours of Extended Leave are accrued for each 260 hours worked.

  • Holidays - Celebrate the Holidays!:

    At Trinity Health we observe the following six legal holidays. New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. Eligibility is after 90 days. Holiday pay is built in to PTO accrual.

  • Health Insurance - We've Got You Covered:

    Trinity Health offers employees health insurance through Blue Cross-Blue Shield of North Dakota. Employees working at least 20 hours per week are eligible for health insurance coverage through Trinity Health.

    Trinity offers a single, single plus dependant, and family health care plan with Trinity sharing in the cost. Eligibility is the 1st or 16th of the month following date of hire, whichever is closest.

    Preventive services covered in addition to your $200 preventive maximum - deductible/coinsurance waived (mammogram, pap smear, fecal occult blood testing, prostate cancer screening services - frequency of services may vary by age group)

    Immunizations covered at 100%

    Dependent Eligibility:
    Eligible Dependent Children under age 26 who are not eligible to enroll in an employer sponsored health plan other than a group plan of the parent may continue coverage until the end of the month in which the children become age 26. The member must notify employer if the dependent children become eligible to enroll in another employer sponsored plan. For additional information, please contact Human Resources.

  • Dental Insurance - Something to Smile About:

    In addition to its health benefits package, Trinity Health offers employees dental coverage under their group health contract. Employees may also obtain family dental coverage for an additional charge.

    Dental coverage begins the same time your health benefits begin. Dental coverage through Trinity Health may also be obtained as a stand-alone benefit, even if another insurance carrier is providing your health insurance. Eligibility is the 1st of 16th of the month following date of hire, whichever is closest.

    Premiums (per pay period):
    Single = $4.50 per pay period
    Family = $23.83 per pay period

    Cleaning Co-pay: $10/cleaning (4 per year)
    Diagnostic and Preventative: 2 cleanings annually, deductible waived
    Restorative: Covers 80% on fillings, 50% on crowns
    Deductible: $50 per person, $100 family
    Benefit Max per year: $1,000 per member

  • Life Insurance:

    After one year of employment, Trinity Health employees, working at least 20 hours per week will receive life insurance benefits equal to 1 ½ times their base pay at no cost to them.

  • Long Term Disability Insurance:

    Following one year of continuous employment, Trinity Health employees working at least 20 hours per week are covered by long-term disability insurance at no cost to the employee. A benefit of 60% of monthly income, not to exceed $3,000, continues until the age of 65 or until the employee returns to work or the disability ceases to qualify the person for coverage.

  • Tuition Assistance - Education is Important:

    Trinity Health provides tuition assistance up to $500.00 per school year to all employees working at least 20 hours per week. Courses must be job-related and approval is required before class registration. For details, contact HR.

  • Scholarships:

    Trinity Health Foundation offers many nursing scholarships each year for both RN's and LPN's. Any student who is accepted into a nursing program is eligible to apply for the scholarship/ loan program. The program may allow for up to $2,000 per semester per student PLUS a $250 stipend each month is employed while attending school.   • Direct Deposit - Convenience For You:

    Trinity Health will automatically deposit your paycheck every two (2) weeks into either a checking or savings account of your choice. Employees will receive a pay voucher each payday reflecting their earnings for the current pay period and year to date.

  • TriniPlus 401(K) - Investing With You:

    Employees of Trinity Health, age 21 or older, are eligible to participate in Trinity's 401(k) retirement plan. Our TriniPlus 401(k) plan allows employees the chance to invest on a tax-deferred basis for long-term goals, including retirement. Even better, Trinity will immediately begin to contribute funds to this program for those employees participating. Trinity matches 25% on the 401k, up to 6% of what the employee contributes. The 401k contributions are fully vested after 1 year. Employees can generally withdraw funds when they leave Trinity Health or reach age 59½.

  • Pension Plan - Saving For You:

    Trinity Health's TriniPlus pension plan is open to all employees after one year of employment, providing they are 21 years of age or older and work a minimum of 1,000 hours per year. This defined contribution plan is fully funded by Trinity Health and employees are fully vested after 6 years of vesting service with Trinity.

  • Triniflex - Flexibility For You:

    Triniflex is available to staff who work a minimum of 1,040 hours per year. This benefit puts additional dollars in employees' pockets by letting them set aside dollars on a pre-tax basis to pay for out-of-pocket medical expenses or dependent adult or child care expenses. Out-of-pocket medical expenses include co-payments, deductibles, prescription drugs, chiropractic care, eyeglasses and out-of pocket dental expenses. In 2011, you are able to flex $7,500 for medical spending (e.g., co-pays, deductibles, co-insurance, dental, vision, prescription drugs, hospital services, lasik eye surgery) and $5,000 for dependent care/daycare expenses for children up to age 13.

  • Voluntary Benefits

    Short Term Disability / Voluntary Income Protection: Trinity Health employees who work a minimum of 20 hours per week, and are under the age of 65, may purchase short-term disability at a group rate. Benefits are paid for a disability resulting from an accident or illness, work related or not.

    CancerCare Elite Insurance: Trinity Health employees who work a minimum of 20 hours per week, are under the age of 65, and qualify, may purchase this voluntary, pre-tax insurance at a group rate. This insurance provides various benefits to a cancer patient including family lodging, airfare, experimental treatment, wig and hairpiece, at home recovery benefit and many more benefits that health insurance does not cover. It also covers a $75 per person/per year Wellness Benefit. This insurance is portable.

    3. Hospital Indemnity PlanTrinity Health employees who work a minimum of 20 hours per week are eligible to enroll in this voluntary post-tax benefit that pays members up to $1000 when they are hospitalized for 24 hours. This insurance is portable.

    4. Long-Term Care InsuranceAll Trinity Health employees and family members are eligible to apply for this voluntary benefit which may help if you become disabled due to an injury, a chronic medical condition, the effects of aging or a cognitive impairment. This insurance is portable.

  • Trinity Discounts - Enjoy!

    Employees, along with their spouse and dependents are provided certain medical service discounts by Trinity Health also. Discounts may include the following:

    1) The difference in charges between a private & a semi-private room after the health benefit payment.

    2) Up to a 50% discount on any hospital balance or surgery center facility fee remaining (not physician fees or services) after the health benefit payment has been made, if applicable, up to a limit of $300 per occurrence. This discount shall be limited to the hospital in Minot and Kenmare only. Discounts will not be extended to take-home drugs, supplies or physician fees. The Surgery Center discount applies to Health Center-Medical Arts Same Day Surgery, or Trinity Community Clinic-Western Dakota Surgery Center.

    3) Maximum discount may not exceed 20% of total charges per occurrence.

    4) The discount may not apply to certain non-covered items (e.g., insurance denied procedures, cosmetic surgery, elective or not medically necessary procedures, etc).


    Trinity also has a 20% discount at Vision Galleria, 20% discount off durable medical equipment, 10% cafeteria discounts, discounts on over-the-counter retail items, discounted movie tickets and car washes, etc.

  • Cafeteria Discount - Eat Up!:

    All Trinity employees enjoy a 10% discount on all food items from the centers cafeterias.
